Mechanism Of Action

Xylometazoline A nasal vasoconstricting decongestant drug which acts by binding to the same receptors as adrenaline. It is applied as a spray or as drops into the nose to ease inflammation and congestion of the nasal passageways. It binds alpha-adrenergic receptors to activate the adrenal system which causes systemic vasoconstriction, thereby easing nasal congestion Xylometazoline is a direct acting sympathomimetic adrenergic alpha-agonist used to induce systemic vasoconstriction, thereby decreasing nasal congestion. The sympathomimetic action of xylometazoline constricts the smaller arterioles of the nasal passages, producing a prolonged (8-12 hours) decongesting effect. In producing vasoconstriction, the result is a decrease in blood flow in the nasal passages and consequently decreased nasal congestion. The drug works by constricting the blood vessels in the nose. The vasoconstriction means that there is less pressure in the capillaries and less water can filter out, thus less discharge is made.
Xylometazoline is an imidazole derivative which is designed to mimic the molecular shape of adrenaline. It binds to alpha-adrenergic receptors in the nasal mucosa. Due to its sympathomimetic effects, it should not be used by people with high blood pressure, or other heart problems.
Benzalkonium chloride is highly soluble in water, it is a quaternary ammonium (cationic) antiseptic These are detergents and have bactericidal, fungicidal properties. They act by altering permeability of cell membranes and denaturing of bacterial proteins. They are non irritating and mildly keratolytic. They are widely used as sanitizers, antiseptic and disinfectant for surgical instruments etc. A 1:1000 solution is used for sterile storage of instruments and 1 in 5000 to 1 in 10,000 for douches, irrigation etc.
Benzalkonium chloride solution is an aqueous solution of a mixture of alkylbenzyldimethylammonium chlorides, the alkyl groups having chain lengths of C8 to C18.

Pharmacology

Pharmacodynamics:
The combination is usually well tolerated in nasal decongestion.
Pharmacokinetics:
Xylometazoline is a direct-acting sympathomimetic which has a vasoconstrictor effect by reducing swelling and congestion when applied to mucous membranes.
Onset of action 5-10 minutes, Duration stays for 10 hours.

Contraindications

Xylometazoline (HCl) is contraindicated in conditions like Narrow-angle glaucoma, Vasomotor rhinitis,Atrophic rhinitis.
It may be contraindicated in patients who are hypersensitive to any ingredient.


Warning and Precautions

Use topical decongestant (xylometazoline) only in acute conditions, not longer than 3 to 5 days. It should be used sparingly in all patients, especially in infants, children and in cardiac patients.

Undesirable Effects

The severe or irreversible adverse effects of Xylometazoline (HCl), which give rise to further complications include Rhinitis medicamentosa.

Overdosage

Overdosage may lead to headache, insomnia, tachycardia, hypertension, nervousness, nausea, dizziness, palpitation, arrhythmia. Nasal drops: Local stinging or burning, sneezing, dryness of mouth and throat, nausea. Eye drops: Dryness of eye.
